How it works

Seen in crypto audits since 2025, the Historical Digital Asset Form asks a taxpayer under examination to list every wallet address and exchange account they have controlled, when each was opened and closed, and the history of activity in each. It is issued during the audit, not filed with a return, and the examiner compares the answers to on-chain data and broker reports.

Why it matters

An incomplete disclosure in an audit is treated as concealment, so the response should be assembled with counsel from a full reconciliation, not from memory.
